By signing this document, I represent that I am the parent/legal guardian of the participant(s) listed below, or that I have obtained permission from the parent/legal guardian of the participant(s) listed below to execute this agreement on their behalf.
I give permission for the participant(s) listed below to jump and play in the inflatables during The First Tee of Fresno’s event held on the date listed above. I am aware and understand that all activities associated with the inflatables at this event involves risk, which I knowingly and voluntarily assume.
I, my heirs, executers, administrators, and assignees waive, release and discharge The First Tee of Fresno, its officers, directors, trustees, employees, and any other personnel officially connected with all activities that myself or the participant(s) listed below may be involved in from all claims of damages, demands, causes of action or suit, and liability of any nature, including claims of negligence arising from or out of the aforementioned activity.
I fully realize the hazards of participation in activities of this type and voluntarily assume all the risks associated with such participation. The participant(s) listed below have the ability and agree to follow all directions from the designated staff and spotters. I assume any medical and emergency expenses in the event of an accident, illness or other incapability that results from participation in this activity.
Furthermore, I agree to remain on the property and in the immediate vicinity at all times while the participant(s) listed below are engaged in the activities related to the inflatables.
